The QR Code is found on the entrance to this road tunnel in east Pawnee.
Note: The map marker for this QR Code doesn't appear until later in the story, though you can still scan it at any point.

Head north up the bridge construction until you find a camera to hack behind a red container.

Look up and right to scan the QR Code.
