As you make your way through the tunnels, you'll happen upon the body of a luckless bugger whose bloody entrails strewn across this short corridor will give you a hand in solving the puzzle. At his feet is a Document.

Eventually, you'll be back in the Asylum. When you arrive, take a look on the floor behind you to find the Map Fragment.

After your chat with the doctor, you'll arrive in a large, circular room. The personal document is on the bed in front of you.
